Access+ Problem Reporting Form

Since we now have to login to get to the form for reporting problems with Access+ PCs, the form automatically fills in the email address of the staff person who logs in. I would prefer that the email address in the 'staff' box on that form be mine; and until the browser session is closed, the last staff person who logged into the form will continue to have her or his email address in that box.

Please take the extra step to replace any email address other than mine with mine when you fill out the form. If you wish to get a response from LST about the problem you are reporting, put your email address in the 'client' box.

  • Best practice is to close all browser windows after your Informatoin Desk shift.
