Access+ Problem Reporting Form

The above form is currently not working. I am waiting to hear back from Catalyst about the status of this. In the meantime, here's what they are asking us to do instead:

1. Email catalysthelp@u.washington.edu
2. Put in the subject "Problem: The Access+ PC DS-OUGL-37", except with their library (SUZ, ALN, FST, ENG, etc) and the correct computer number.
